Visit Information

What to Expect

Before a surgical procedure, most patients will visit our office for an examination and an explanation of the procedure and the recovery period. Our entire staff works as a team to answer your questions, make necessary arrangements and assure that you and your family feel prepared.

Over the weeks following your surgery, you will have follow-up visits to assess how you are healing.

Your First Visit

  • Please arrive for your first visit at least 15 minutes prior to your appointment time.
  • If you are unable to keep your appointment, please kindly give us a 24 hour notice.
  • Please bring a list of your current medications, including drug name, dose and prescribing physician.
  • Your insurance company may require you to have a referral from your primary care physician to be seen in our practice — please make sure this referral is in place.
  • We accept most health insurance plans — please bring your insurance card with you. If you are unsure of your insurance coverage policy, call the customer service number on the back of your card to verify physician participation.
  • We ask that you make payment for co-payments at the time of service. If you do not have your co-payment at the time of service, you may be asked to reschedule your appointment. Please feel free to discuss your health insurance coverage with the billing specialist.

You may be asked to complete forms before your visit. We provide them here for you convenience.
